Melody Program Grant Application

 

The following pages will take you through the entire grant application for the Melody Program. Please read the information below before starting.

• Your school must have an existing instrumental music program (concert band, orchestra, jazz band, orchestra, etc.) that is at least three years old. Please note that if your program consists only of Orff instruction, it is not eligible to apply.
• Please provide information for the applying school only (schools must apply separately, please do not provide information for other schools within the district even if the music teacher is the same.)
• The application consists of several sections, with one section per page.
• You may skip any section and proceed to the next page if you do not have the information at hand to complete that section, but you must return later to complete the section before we can consider your application.
• You may stop at any time, and resume whenever it is convenient by simply logging in again.
• Our system will permit you to review your previous answers at any time prior to final submission of your application, and return to any section to review or change those answers.
• Once all sections have been completed, our system will allow you or others in your school or program to review all information provided prior to final submission of the grant application to The Foundation.

 

Some general notes on completing the application:

• Please answer all questions thoroughly.
• Many questions ask for specific demographic and financial information. Please be as accurate as possible.
• Often a space is provided for you to provide comments or notes on your responses to specific questions. Please make use of these spaces to help us better understand your needs and situation.
• Finally, please double-check your application prior to final submission. Our staff and review board process a large number of applications each year, and your chances of a timely and positive review depend on the thoroughness and accuracy of your answers.
